LexisNexis offers the All New York Forms file. Find this source by following: Legal > States Legal - U.S. > New York > Search Forms & Drafting Instructions > All NY Forms Premium LexisNexis Forms.

You have the option of searching the full text or the enhanced Table of Contents (TOC). I recommend using the TOC to quickly retrieve the most relevant forms for your topic. For example, search within the TOC using these terms: incorporation w/3 corporation and amendment. As of this writing, the search yields 12 possible forms.
